However, Caine and Marquis' soldiers launch a relentless assault on the sanctuary, mercilessly slaying its manager, Shimazu Koji (superbly portrayed by Hiroyuki Sanada). Though John shared a bond with Koji in the past, Koji's daughter, Akira (played by the talented Rina Sawayama), remains convinced that John still owes her late father a debt. Upon returning to New York, John is informed by Winston that he must earn a seat on the High Table by challenging Marquis to a duel, representing a powerful crime family.
John's journey takes him to Berlin, where he seeks to initiate a duel on behalf of the Ruska Roma syndicate. While his adoptive sister, Katia (portrayed by Natalia Tena), harbors deep resentment toward him, John manages to win the family's favor by eliminating Killa (brilliantly played by Scott Adkins), a prominent member of the German High Table. This encounter also sheds light on John's friendship with Caine, who finds himself compelled to work for Marquis in order to protect his daughter's life.
John successfully challenges Marquis to a duel, with Winston acting as his second, while Marquis selects Caine. The long-awaited duel commences, with John and Marquis standing thirty paces apart, their pistols poised to decide their fates. Each gunshot brings them ten paces closer, heightening the suspense. Surprisingly, both adversaries manage to elude death, enduring several grueling rounds. In a climactic turn of events, Koji takes aim and delivers a shot that gravely wounds John, causing him to collapse on the ground. But before Koji can deliver the final blow, Marquis seizes the opportunity to execute John himself. Unbeknownst to Marquis, John's gun remains unfired. Displaying supreme resilience, John seizes his chance and delivers a fatal strike to Marquis, emerging as the triumphant victor of the duel.
The High Table's Harbinger (masterfully portrayed by Clancy Brown) reinstates Winston as the revered concierge of the reconstructed New York Continental. Meanwhile, John Wick, gravely wounded, collapses on the steps of the church, seemingly lifeless. In a poignant scene, his funeral takes place in New York, where he is laid to rest beside his beloved wife, Helen (portrayed by Bridget Moynahan).
